This is one of the most common sources of confusion for people monitoring their blood glucose — and one of the least well explained by healthcare teams.
You've eaten nothing since dinner. You've been asleep for eight hours. You wake up and test your glucose — and it's higher than you expected. Sometimes higher than it was after lunch. This doesn't make sense on the surface. But there's a straightforward explanation.
In the early hours of the morning, the body prepares for waking up. Hormones — primarily cortisol and growth hormone — are released as part of your natural circadian rhythm. These hormones signal the liver to release stored glucose into the bloodstream, providing fuel for the brain and body as you transition from sleep to waking.
In someone without insulin resistance, insulin quietly manages this glucose release and blood sugar stays stable. In type 2 diabetes, that insulin response is impaired — so the glucose released by the liver builds up in the bloodstream, and your morning reading comes out elevated.
This is the dawn phenomenon, and it is hormonal. It is not caused by what you ate the night before. A perfect dinner will not prevent it.
Less commonly, high morning glucose can result from the opposite problem. If blood glucose drops too low overnight — most often due to insulin or sulfonylurea medication — the body responds by releasing stress hormones that trigger a sharp glucose release from the liver. The result is a rebound high by morning.
The Somogyi effect is most relevant for people on insulin or gliclazide. It can be identified by testing glucose in the middle of the night — a very low reading followed by a high morning reading suggests Somogyi rather than dawn phenomenon.
The distinction matters because the management is different. Dawn phenomenon is best addressed through lifestyle factors — meal timing, evening movement, and in some cases medication adjustment. Somogyi requires a review of medication doses, which should always involve your doctor or pharmacist.
Fasting glucose contributes significantly to your overall average glucose levels. If your morning readings are consistently elevated due to dawn phenomenon, it will pull your HbA1c upward even if your after-meal readings are well controlled. Addressing it specifically can make a meaningful difference to your overall glucose management.
